Orthognathic Surgery: A Step-by-Step Approach to Dentofacial Deformity Correction

Orthognathic surgery serves as a foundational element for correcting dentofacial deformities. While these procedures are reliable and safe, proper preoperative assessment is crucial to achieving optimal patient outcomes. Diagnosis remains the most critical step in the clinical management of any patient. As the saying goes, “A journey of a thousand miles begins with a single step.” The aim of this paper is to explore the step-by-step pathway of a patient destined for orthognathic surgical intervention. The methodology employed is based on a review of the global literature on Oral and Maxillofacial Surgery, particularly focusing on Orthognathic Surgery, to compile a comprehensive outline of preoperative assessment and management, including specific diagnostic phases during the orthognathic pre-surgical period.Conclusion: A detailed medical history, accurate clinical examination, use of radiographic and photographic imaging for diagnostic purposes, model analysis, specific orthodontic treatment, surgical modeling, and virtual surgical planning remain the most important components for achieving success in orthognathic surgery. There is much to discuss in terms of scientific and artistic perspectives, as well as methodological approaches to achieve aesthetic facial objectives, restore dental health, and enhance head and neck function through the reconstruction of dentofacial deformities. Ultimately, the goal is to create facial proportions that yield an aesthetically pleasing appearance for the patient, regardless of the viewpoint.
